Community based cancer patient navigation
ACRSS patient navigators are trained to provide culturally and linguistically specific services and to offer support and guidance for patients, families, and caregivers throughout their cancer journey. Patient navigators can assist each patient and their loved ones to ensure that their needs are met and connect them with information and resources, peer support group, and counseling in a safe and caring space. The program provides individualized care to meet the unique needs of the patients and caregivers.
Patient navigation services will increase access to care, improve treatment outcome and enhance quality of life.
